CASE CERTIFICATE
REQUIREMENTS
1. Application Form ;
2. Copy of Registration Under Rule 1020 of OSHS ;
3. Copy of Registration Under D.O. 174 or PCAB License or PEA License, if applicable ;
4. Copy of government-issued ID (of the requestor) ; and
5. Copy of Company ID (of the requestor).
